Behavioral Health Ages 4–18

Routine mental health screening at every well visit. In-office support, care coordination with therapists, and warm handoffs to behavioral health specialists.

Mental Health All ages

M-CHAT screening at 18 and 24 months. ASD care coordination, therapy referrals (ABA, speech, OT), school advocacy letters, and ongoing medical management.

Personalized written asthma action plans, controller and rescue medication management. Nikki Moundros leads our asthma program with school coordination included.
